QUALIFYING DOCUMENTS
Provide documentation for one or more of the state-based criteria below:
 
Eligible for National Free or Reduced-Price Lunch Program - Limited Income ( students do not automatically qualify for meals - applying for meal benefits is a separate process )
  • SNAP or TANF eligibility letter
  • Three most recent paycheck stubs for every working members of the household
  • Three most recent pay stubs for Unemployment, Worker’s Compensation, or Disability payment for every adult in the household, if received
  • Letter from employer stating hours worked, how often paid, and rate of pay, if paid in cash 
  • Last year's tax return - valid only if you are self-employed

Child or Step-Child of an Active Duty Member of Armed Forces or Who Was Injured Or Killed While Serving on Active Duty
  • Department of Defense (DoD) photo ID
  • A statement of service from the installation adjutant general director of human resources for children of active members, mobilized reservist, or members of the Texas National Guard
  • For children of service members who died or were killed, a copy of the death certificate using the service-appropriate DoD form or a DoD form that indicates death as the reason for the separation from service.
  • A copy of Purple Heart orders or citation for children of service members, mobilized reservist, or guardsmen who were wounded or injured in combat
  • A copy of the line of duty determination documentation for children of service members, mobilized reservists, or guardsmen who were injured while serving active duty but were not wounded or injured in combat.
 
Foster Care - Is, Or Has Ever Been, In Conservatorship of Dept. of Family and Protective Services
  • Form 2085-Foster Care, court adoption records as a result of DFPS custody or proof of DFPS conservatorship (including children returned to parents after hearing) or official letter from DFPS or court. 
 
Child of Star of Texas Award Recipient
  • The resolution (certificate) awarded to an individual serves as proof of eligibility to enroll these children in free prekindergarten.
 
English Learner/Emergent Bilingual Student or Homeless
  • PreK will follow up after reviewing online application 
 
ENROLLMENT DOCUMENTS
For faster registration, upload original copies of the following items:
 
  • Student Identification (Certified Birth Certificate - state issued)
  • Social Security Card (requested)
  • Student's Immunization Record (must be up to date at the time of registration)
  • Proof of Residency
    • If living in one's own residence or renting - provide rental agreement/lease or utility bill
    • If building in Weatherford ISD - provide builder's contract showing projected completion date and utility deposit
    • If living in another person's residence - both parent/guardian of student and the person who lives/owns the residence will need to sign a notarized Affidavit of Residency form (see attachment located at the right of this page.) 
  • Parent Identification
    Valid Photo ID; Driver's license, Passport, Permanent Resident Card or Naturalization Papers
  • Custody information, if necessary
