Conflict of Interest Questionnaire - Form CIQ

Texas Local Government Code Chapter 176.003 requires vendors or potential vendors of the District to fill out and file a Texas Ethics Commission Conflict of Interest Questionnaire IF the person has a business relationship with the District AND:

 

1. Has an employment or other business relationship with an officer of the District, or a family member of the officer, described by Local Government Code 176.003(a)(2)(A);

- OR -

2. Has given an officer of the District, or a family member of the officer, one or more gifts with the aggregate value specified by Local Government Code 176.003(a)(2)(B), excluding any gift described by Local Government Code 176.003(a-1).

 

 If neither of these conditions exist, it is NOT necessary to file this form.

 

3. Forms will be removed once there is no longer a business relationship with the vendor or the Governmental officer is no longer with the District.

 

IF a form is required, it must be filed with the District's Procurement Department, and can only be filed by scanning and emailing to [email protected]

 

 Note: Bid/Quote responses should be sent to the Purchasing Department at the physical or email address listed on the Bid/Quote form.

 

This questionnaire must be filed no later than the seventh day after the date that the vendor (1) begins contract discussions or negotiations with the district; or (2) submits to the district an application, response to a request for proposals or bids, correspondence, or other writing related to a potential agreement with the District. It is a Class C misdemeanor to fail to file the conflict of interest questionnaire.

 

In accordance with Chapter 176 all questionnaires must be posted on the District's Web site.  If you have questions regarding the filing of this form, you should contact your lawyer or other legal representative.
